Back-to-School Health and Wellness/Salud y bienestar para el regreso a clases

If your student has diabetes, asthma, seizures, or a severe allergy, access to their prescribed treatments and medication is essential to a healthy and safe return to the classroom. Learn about reporting a chronic condition to your school and the accommodations available to students by viewing the videos on this webpage. 

If your student has a chronic condition, follow these four steps ahead of the new school year:

●Access the required forms for your student’s health condition by visiting cps.edu/medicalforms or speaking with our school nurse.
● Ask your student’s medical provider to complete and sign the forms.
● Bring the signed forms and medication to our school nurse for review.
● Work with our school to complete a 504 Plan/IEP or Emergency Action Plan. This will ensure our staff is prepared should an emergency arise.

By following these four simple steps, you can ensure your child has a healthy and safe start to the new school year.

Some other ways to look after your whole family’s health this school year:

Redetermine Medicaid – Complete the redetermination process each year so you don’t lose your Medicaid benefits.

Immigration Health Benefits – Check to see if you qualify for the new Illinois health insurance program for immigrants age 42+.

COVID-19 Vaccine and Boosters – All students are now eligible to receive the COVID-19 vaccine. Check with your pediatrician or visit cps.edu/vaccinations to make an appointment.

Si su estudiante sufre de diabetes, asma, convulsiones o de una alergia grave, el acceso a los tratamientos y medicamentos prescritos es indispensable para poder regresar al salón de clases de forma saludable y segura. Obtenga información sobre cómo reportar a la escuela una condición crónica y las adaptaciones disponibles para el estudiante al visualizar los videos en esta página web.

Si su estudiante presenta una condición crónica, antes del inicio del nuevo ciclo escolar, siga estos cuatro pasos:

●Acceder a los formularios requeridos para la condición de salud del estudiante visitando cps.edu/medicalforms o hablando con la enfermera escolar.
● Pedir al proveedor médico del estudiante completar y firmar los formularios.
●Llevar los formularios firmados y el medicamento a la enfermera escolar para revisión.
●Colaborar con la escuela y completar un Plan 504/IEP o un Plan de acción de emergencia. Esto asegura que nuestro personal esté preparado en caso que se presente una emergencia.
